Thoreau Health Office - Fourth Grade Physical Exams

All students entering fourth grade must have a current physical examination carried out after January 1 of the entry year. A copy of the examination results must be on file in the Health Office. Notification of this physical examination requirement will be mailed to all grade three students after

January 1.
